Karachi’s densely populated neighbourhoods, chronic traffic jams and narrow lanes often delay conventional ambulances from reaching patients during medical emergencies. To improve response times in such locations, the Sindh Integrated Emergency and Health Services (SIEHS-1122) introduced a bicycle ambulance pilot project in May 2025.

Equipped for Emergency Response

The bicycle ambulances are operated by trained emergency responders who can reach incident sites ahead of standard ambulances. Their role is to provide immediate first aid, stabilise patients and deliver essential emergency care until a fully equipped ambulance arrives.

Rather than standard bicycles, the units use specially modified mountain bikes carrying a range of medical equipment, including:

  • First-aid supplies

The programme also features the active involvement of trained female volunteers, expanding access to emergency healthcare while encouraging greater participation of women in frontline rescue and medical services.

Mixed Reactions Online

The viral videos have generated thousands of comments, memes and humorous posts on social media. Some users joked that while other countries are investing in air ambulances and advanced healthcare technologies, Pakistan is relying on bicycle ambulances. Others sarcastically labelled the project as the country’s “latest innovation.”

Despite the criticism, healthcare professionals and many social media users have defended the initiative. They argue that in densely populated urban areas where conventional ambulances are frequently delayed by traffic or inaccessible streets, bicycle ambulances can provide a faster first response. Supporters say the model offers an efficient, affordable and potentially life-saving solution for emergency medical services in challenging environments.
